Documents to prepare
Along with the signed and stamped hard copy, the medical report and relevant leave documentation, duly signed and stamped by the HOD, must also be submitted. Without these documents, the data will not be finalized.
Important to know
The data submitted by a department will be considered FINALIZED only after submission of the HARD COPY, duly signed and stamped by the HOD on each page, in the O/o District Informatics Centre, NIC, Room No. 103, First Floor, District Administrative Complex, Sangrur. Any discrepancy, missing information, or incomplete documentation may result in the data not being finalized until the required corrections/documents are submitted.
