This form, complete in all respect, must be submitted to the concerned Teaching Department /Centre/ Institute (copy to dis@pu.ac.in) as has been indicated in the Admission Notice for the session 2026-27 as well as in the Handbook of Information 2026 on or before the last date indicated at the top of admission form, even if, result of the qualifying examination has not been declared.
